If you're a new client, please provide your email address to (909) 280-2833 and a link for the online patient portal called Simple Practice will be emailed to you. Intake documents will be sent via the patient portal for you to complete online in preparation and prior to your first therapy session.
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete ask for a Release of Information Form to be sent through the online patient portal permitting therapist to authorize release of psychotherapy information, gather information from another provider or person, and/or collaborative with another party for holistic treatment or care.
