Rates
$250 per 45-minute session
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Reduced Fee
Reduced fee/sliding scale services are available on a limited basis. Special arrangements must be made with the therapist directly.
Payment
Cash, check and all major credit cards accepted for payment. Please make checks payable to Melissa L. Zwart, LMFT.
Cancellation Policy
I have a 48 hour cancellation/reschedule policy. You are responsible to pay weekly for your therapy session unless you cancel within a 48 hour window. Cancellation should be done by phone at (909) 280-2833. Calls are time and date stamped.
